Set "needFallThroughBranch" flag during block split.
Bonus changes:
* Add a undefined Thumb instruction between the last code block and the
beginning of PC reconstruction cell to capture such codegen problem on
the spot.
* Fix a loop formation problem to exclude nested loops.
