Remove usages of CallerInfo

Remove usages of CallerInfo and CallerInfoAsyncQuery from the old
telephony placeCall method -- the method is only used by OTASP these
days and has no need for caller info.

Also removed CnapTest because the method it was testing has no other
callers.
